Most likely CVI make as it has Continuum part# Appears to be BK7 or nicer substrate. Hard to take pic of the AR coating on the long face. It's pinkish color tells me it's for 532nm or blue wavelengths. 55mm long and cross section of 15mmx15mm.Thanks for looking
