Yaskawa Servopack model SGDA-A3CSY83. 24 volts@ 2 amps. The motor is an SGM-A3C3NQ22, rated at 0.095 nm, 30w/24 volts, 1.6 amps. Has a very nice precision coupling on it and a bearing guided linear screw. HAs a high quality encoder on the rear of the motor. Also supplied is the connecting cable from the motor to the drive. It is about 10 feet long or so.
