Catalog Number- AD 4150 Input: 120 volts 60 Hz 12 watts Output: 4.5 volts DC 1 amp On the plug the inner contact is negative and the outer contact is positive. Made In U.S.A I tested this adapter with a multimeter and it works. On the bottom of the adapter there's a hole where the wire comes out. I think there was a protective rubber cover of some type but it's not there now( see picture 3) This doesn't affect the operation of the adapter. If you have any questions just ask.
