It looks like the shank face opposite the insert pocket(bottom of holder) and the shank face to the right of the insert pocket(face that would seat against the tool holder block in your machine) were where the alterations were made. The shank is supposed to be 1-1/4” square but it is measuring about 1.24” by 1.01” The insert pocket is in excellent condition.
